The Prince Albert Parkland Health Region has programs that assist people who have chronic illnesses, such as diabetes, heart and pulmonary disease.

The increasing demand to deliver therapeutic and preventive care sharpens the need for disease management strategies that accomplish these goals efficiently while preserving quality of care. The Region’s multidisciplinary team offers the best and most current evidence based clinical practice. The clinic operates Monday through Friday 8 a.m. to 5 p.m.. Individuals can self-refer to the clinic or be referred in by any health professional. Services include:
- Individual appointments with clients to assist them in attaining the skills needed to live healthy with diabetes.
- A foot clinic, which not only provides needed care and treatment, but also strives to teach skills for “healthy feet”.
- A one-day multi-disciplinary “Survival Skills” class. This class provides education to individuals with diabetes, teaching them the required skills to live healthier lives.
- Initiate and adjust insulin to assist clients in achieving optimal glycemic control
- Insulin pump trainers on staff who assist pump clients to initiate and maintain insulin pump therapy.
- The Region has certified pump technicians on staff to initiate and titrate basal rates and bolus to achieve optimal glycemic control.
- Services are for individuals who are “pre-diabetes”, Type 1 Diabetes, Type 2 Diabetes, Gestational Diabetes and pregnancy with pre-existing Diabetes. The dietitian also does counseling for any other outpatient dietary referrals.
- A monthly Heart Healthy class is held. Education is provided to at-risk and post-cardiac clients.
The Centre also acts as a resource for all professionals within the Health Region. And we also work closely with the family physicians in Shellbrook, providing them with an on-site clinic at their offices once a week.
